Instructions for Client or corporation name changes

A name change request should be submitted if any of the following occur:

  • marriage (and the Client changes their name)
  • return to maiden name
  • legal name change (Archibald Leah legally changed his name to Gary Grant)
  • name is incorrect on our records
  • adoption (and the Client changes their name)
  • Client is commonly known by another name.

Note: Both the Client's legal and commonly-known names should be included in their file.

How do you request a name change?

For Individual Insurance and Individual Wealth (Insurance GICs & Trust GICs)

Complete form E85 - Name change or correction.

Make sure the Client dates and signs the form, indicates the reason for the change, and provides the necessary documentation to support the change.

What supporting documentation is required?

We will accept a copy of the supporting document.

Reason for name change Supporting documentation required
Marriage Marriage certificate or driver's license showing the new name.
Return to maiden name Birth certificate or driver's license showing the maiden name.
Legal change Supporting documentation of change
Current name was entered incorrectly in our records

Birth certificate or document of proof

For example, if our records show the Client's name as Jean Doe, but the name really should have been Jane Doe, we require proof that Jean Doe is the same person.

Adoption Adoption order document or new birth certificate
Client is commonly known by another name

For example, if the Client's legal name is David Wells, but he goes by Boomer, we can change his mailing name to Boomer Wells, but need the birth certificate to prove that Boomer isn't his legal name. The legal name does not change.

What if the name change is for a corporation?

If the name of a corporation is changing, a Certificate of Amendment is required. To transfer ownership between companies, please complete form  E82 - Transfer of ownership.
