Previously, we explained that in-force Clients with optional dental coverage on PHI Standard or Enhanced and HCC B or C for Series 6 to 9 plans could request the removal of the dental coverage by submitting Form 4366 – Remove optional benefit.

We are pleased to share that we have expanded our process to also allow in-force Clients to submit Form 4366 if they wish to remove the built-in dental portion from their PHI or HCC plan. This consistent process provides PHI and HCC Clients with more flexibility and provides you with another solution to help meet a Client’s changing needs.
And more good news! To help streamline the process, as of October 21, 2024, Clients can also call 1-800-SUN-LIFE (1-877-786-5433) directly to remove the benefit.
In all situations, Clients must consider their options carefully. If a Client would like to remove the optional or built-in dental coverage from their PHI or HCC plan, inform them that once they remove the dental portion they cannot add it back in to the plan. Clients should also note that there may be a co-payment required with the CDCP.
