Updates to Name change forms coming June 22

To remain compliant, we’re updating our Name change or correction forms (form 85 for insurance and form 324 for Personal Health Insurance) on June 22.
This change will help us align with regulatory requirements. The form now asks for supporting documentation when a Client changes their name due to marriage or if they return to their maiden name.

Going forward, Clients will need to provide the following to remain compliant:
| Reason for change | Documentation needed |
| Marriage | A copy of their marriage certificate or driver’s license |
| Returning to maiden name | A copy of their birth certificate or driver’s license |
