Diabetes Care Program

Life insurance Clients1 with type 1, type 2, or pre-diabetes may register for the Diabetes Care Program – a complimentary program of wellness offerings to help Clients manage diabetes. The Diabetes Care Program is available through Lumino HealthTM  Pharmacy (LHP), with service provided by Pillway2.

The following information is for your awareness only. The program will be offered to Clients directly from Sun Life after the policy is issued. The Diabetes Care Program (DCP) is a voluntary non-contractual offering. Sun Life reserves the right to terminate, suspend or modify the Diabetes Care Program offerings at any time and without notice. 

The DCP offer will depend on the Client’s location and the type of diabetes they’re living with. We’ll send messages to eligible Clients to register for the Diabetes Care Program, including a unique referral code they’ll need to use on the registration page to access the offerings.

1 The Diabetes Care Program is available for Clients between 18 and 65 years of age who have a Sun Life Evolve Term, Sun Life Essential Term, Sun Life Achievers Term, Sun Life 1-Year Bridge Term, or Sun Permanent Life insurance product.

5The trial kit is only available for Clients who have not previously used Abbott's FreeStyle Libre 2 sensor and are diagnosed with type 1 or type 2 diabetes.
